Restylane and Juvederm
Synonyms - filler substances, injection replacement
Indications
These are all used to soften facial lines (especially the fold between the corner of your mouth and nose = 'smile line') and enlarge the lips. Restylane and its versions are temporary, while Artecoll and Dermalive are permanent. A recent trend is to use the fillers more extensively in the face to replace volume that we have lost in the aging process. Volume replacement over the cheekbones, under the eyes, along the jawline below the mouth and temples gives a more youthful appearance.
Hands can also be rejuvenated by adding a thin layer of filler over protruding veins and bones that become more prominent as we mature.

Collagen
Collagen was the gold standard filler substance for many years and a name many are familiar with. It is temporary and requires skin testing to rule out allergy (1-3% incidence). It has been largely replaced by better fillers.
Restylane
This is a synthetic hyaluronic acid. A version has been used safely for years to cushion arthritic knee joints. It is injected into the deeper layer of the skin and molds particularly well to smooth out moderate creases and augment lips. No prior skin testing is required. It is temporary and lasts approximately 2-3 months in the lips and, for many, 4-5 months in 'smile lines'. Overall, duration is variable. Restylane Fine Lines is for fine, superficial lines (i.e. upper lip). Perlane, a denser version, was introduced for correction of deeper grooves, such as those between the nose and mouth corner.
Juvederm
This is also a synthetic hyaluronic acid, similar or longer than Restylane in duration. No skin testing is required. Dr. Lozinski particularly likes to use this product in the nasolabial (nose to mouth corner smile lines).
Is anesthetic required?
Injection of fine and deeper lines around the nose and mouth are better tolerated with local anesthetic. For Restylane Fine Lines correction of fine lines around the mouth the local anesthetic (regional nerve block) is recommended as well but possible without.
What is the recovery period?
Deep crease and fine line areas are minimally swollen that evening and many people will feel comfortable going out that same night. Lips tend to swell more than other areas, but most recover by the next morning. Bruising is uncommon, except with lips and the under-eye hollows, and its incidence minimized by avoiding aspirin and anti-inflammatory agents such as Advil for 2 weeks before.
